We begin by leveraging data gathered through Community Health Needs Assessments (CHNAs). These assessments are the cornerstone of our community assessment strategic planning process, providing invaluable insights into the community’s health landscape. By analyzing this data, we identify priority health issues, trends, and disparities that require attention.

This process includes three key meetings: 

  • Internal All-Staff Meetings: Engaging with all staff members to gather insights and foster a collaborative environment
  • Leadership Team Meetings: Working closely with your leadership to align strategic priorities
  • Board of Health Presentation: Presenting strategic plans to your board for approval and support.

The result is a comprehensive 3-5 years Strategic Plan that serves as a roadmap for public health improvement— to accreditation and beyond!
